29 CFR 1926.651(b)(1):     The estimated location of utility installations, such as sewer, telephone, fuel, electric, water lines, or any other underground installations were not determined prior to opening an excavation.    a) Construction site - On or about May 8, 2012, The employer did not contact the utility company (via PA 1call) prior to commencing the excavation of a clean out pit in close proximity to underground natural gas and electric transmission lines, exposing employees to potential serious injury when the underground natural gas line was struck and ruptured.
