Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    a)  Process and Finishing area:  On or about May 17, 2012, the employer failed to ensure that a Delta drill press, serial no. 8902 was equipped with a guard.  The drill press is used to make plugs to keep coolant from going into pipes.  Employee(s) were exposed to unguarded rotating parts.

Work rests shall be kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the workrest, which may cause wheel breakage.     a)  Machine shop - second building:  On or about May 17, 2012, the employer failed to ensure that the workrest on a Dayton 7" bench grinder (model #2LKR8, lot #120101809) that employees used to grind inserts was adequately adjusted.  The distance from the abrasive wheel and the tongue guard measured 1 1/2 inch on the right wheel.      b)  Machine shop - third building:  On or about May 17, 2012, the employer failed to ensure that the workrest on a Westward 6" bench grinder (serial #110602236) employees used to grind inserts measured 1/2 inch from the work rest to the right abrasive wheel.     Employee(s) were exposed to a struck-by hazard.
