29 CFR 1926.451(b)(1):     Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports.    At a jobsite in Mount Morris, Illinois employees were working from a scaffold approximately 18.5 feet above grade that was not fully planked.    Abatement certification is required of this item in accordance with the requiremts of 29 CFR 1903.19(c)

29 CFR 1926.451(f)(6):     Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6).    A metal scaffold system adjacent to a residential structure was not placed at a minimum of 3 feet from an electrical drop leading into the residence.     There is no certification or documentation required for this item.
