29 CFR 1926.100(a):     Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    a)  1 Prospect Street, Vernon, CT. On or about 6-13-12 - Employees cleaning up roofing shingles and debris were working under the scaffolding system and roof eave while roofing operations were ongoing with no head protection exposing them to falling shingles and other roofing material.

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) 1 Prospect Street, Vernon, CT. - On or about 6-13-12 - Employees engaged in roofing operations were accessing the ladder jack scaffold system with an aluminum extension ladder that did not extend 3 feet above the scaffold platform, 12 feet above the ground.      SRM Custom Roofing & Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard Subpart X - Stairways and Ladders, 29 CFR 1926.1053(b)(1) which was contained in OSHA Inspection Numbers 311762090, Citation Number 1, Item Number 3, issued on 05/19/09 with respect to a workplace located at 83 Linwood Street, New Britain, CT. 06052, which became a final order on 6/15/09.     SRM Custom Roofing & Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard Subpart X - Stairways and Ladders, 29 CFR 1926.1053(b)(1) which was contained in OSHA Inspection Numbers 311782502, Citation Number 2, Item Number 3, issued on 01/14/10 with respect to a workplace located at Wolcott Hill Road, Wethersfield, CT 06109, which became a final order on 04/29/10.
