Meridian Rd, Newton, KS in the maintenance building, the Enco Band saw was missing point of operation guarding on the left and right side of the 1" blade exposure, exposing employees to an amputation hazard.

Meridian Rd, Newton, KS, in Building 3 in the Edge Banding area, a seven-outlet power strip was plugged into a 25 feet extension cord.  The power strip was being used to power a box fan, exposing employees to a potential fire hazard.

Meridian Rd, Newton, KS in the Edge Banding Area, the Holzma Panel Saw electrical control panel door latch was defective, leaving the door open, exposing employees to a shock hazard from the 460 VAC equipment contained inside the enclosure.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D)(h):    In Building 1, the Viet Wide Belt Sander Lockout/Tagout procedures were inadequate for the control of hazardous energy in that the procedures developed did not correctly identify the electrical disconnect on the front of the machine as the proper lockout/tagout location, nor did the procedures address the control of the pneumatic energy on the equipment.
