29 CFR 1910.119(j)(2):     The employer did not establish and implement written procedures to maintain the on-going integrity of process equipment:    a)  Pressure Relief Valves on Propane Storage Tanks - The pressure relief valves protecting propane storage tanks #1 and #2 were not tested in accordance with recognized and generally accepted good engineering practices since their installation on or about 1995 to verify that the valves perform reliably.      b)  Propane Storage Tanks - Propane storage tanks #1 and #2 did not receive an internal inspection in accordance with recognized and generally accepted good engineering practices since their installation on or about 1995 to verify the ongoing integrity and remaining service life of the tanks.    c)  Propane Piping System - The propane piping system was not inspected and tested in accordance with recognized and generally accepted good engineering practices since their installation on or about 1995 to verify the ongoing integrity and remaining service life of the piping.

The employer removed an excess flow valve, replacing it with a Fisher Type C477-1625 valve having excess flow and automatic shutoff features.         2.  The employer relocated a Fisher Type N310-16 globe valve from a position near the bobtail load stanchion to a position near the meter.

29 CFR 1910.119(l)(4):     Process safety information required by paragraph (d) of this section was not updated when a change covered by this paragraph resulted in a change in the process safety information:    The Pine Tree Propane Piping Plan was not updated in regard to the following changes:        a)  Bobtail #2 Loading System, Liquid Line - A Fisher Type N310-16 globe valve was relocated, and an excess flow valve was replaced with a Fisher Type C477-1625 valve which functioned as an excess flow and nitrogen-to-open automatic shutoff valve.      b)  Bobtail #2 Loading System - The methanol piping system was removed and a drain valve was installed where the methanol piping system connected to the propane liquid pipe.
