29 CFR 1910.178(l)(4)(iii):     An evaluation of each powered industrial truck operators performance was not conducted at least once every 3 years:    a- Amalfe Brothers Inc., dba AB & S Warehouse, Edison, NJ - Forklift operators were not re-evaluated at least once every three years after initial training.  Evaluation to include, but not limited to, driving skills, review of safe operations, etc.  The evaluation should be documented to include the name of the operator evaluated, the name of the person(s) providing the evaluaton and the date of the evaluation.    Condition noted on or about 6/13/12.
