This violation was observed on or about June 12, 2012, on the steep roof of a newly built office building where employees were exposed to a 12 foot fall hazard when performing roofing work without a fall protection system.

This violation was observed on or about June 12, 2012, on the roof of a newly built office building where employees were exposed to an approximately 12 foot fall hazard while using a metal extension to access the roof and the ladder was extended approximately 10 inches above the roof.

This violation was observed on or about June 12, 2012, on the roof of a newly built office building where employees were exposed to an approximately 12 foot fall hazard while using a metal extension to access the roof and the ladder was installed with the footing approximately 6 feet from the wall.
