29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas:    a) Leone High School Project B, south side:  On or about 10 MAY 12, piles of scrap lumber with protruding nails were not kept clear throughout the area. Workers on the jobsite were at risk as they were exposed to scrap lumber with protruding nails on the ground.

29 CFR 1926.25(b): Safe means were not provided to facilitate removal of combustible scrap and debris:    a) Leone High School Project B, south side:  On or about 10 MAY 12, combustible piles of scrap lumber with protruding nails, debris such as cardboard boxes, paper and trash were not removed at regular intervals exposing workers to a fire hazard.

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.    a) Leone High School Project B, south side:  On or about 10 MAY 12, employees were observed using extension cord to portable power tools such as Skil Saw and rebar cutter without using ground fault circuit interrupter (GFCI) exposing workers to the hazards of contact electricity.       FOR A REPEAT VIOLATION: WULF CORPORATION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.404(B)(1)(I) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 315436352 CITATION NUMBER 1, ITEM NUMBER 3, ISSUED ON JULY 19, 2011, WITH RESPECT TO A WORKPLACE LOCATED AT LEONE HIGH SCHOOL PROJECT B, PAGO PAGO AMERICAN SAMOA.
