29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation: The employer does not provide scaffold with base plates, mud sills or other adequate firm foundation. This violation was observed on or about June 11, 2012, on the east side of a newly built house where employees were exposed to a fall hazard when required to work on the platform at a height of 10.9 feet without base plates, mud sills or firm foundation.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access. The employer does not provide employees with access to the scaffold platform. This violation was observed on or about June 11, 2012, on the east side of a newly built house where employees were exposed to a fall hazard when accessing the platform without a ladder.
