29 CFR 1926.453(b)(2)(v):  A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   The employer does not protect its employees from ejection while working from an aerial lift. This violation was observed on or about June 11, 2012, located at the work site at 21348 Kuykendahl, Spring, TX, where two employees were exposed to an ejection hazard while working from an aerial lift at a height of approximately 21 feet without a restraint system.
