29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.      a)  At the jobsite, on or about 5/23/12:  Employees were engaged in roofing work on a low slope roof, approximately 17 feet from ground without fall protection of any kind.      Abatement certification must be submitted for this item.

29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a)  At the jobsite, on or about 5/23/12:  Employees were accessing a roof surface, approximately 17 feet from gound, and using an adjustable "little giant" type of ladder which did not extend above the upper surface by at least 3 feet.    Abatement certification must be submitted for this item.
