29 CFR 1910.24(i): Vertical clearance above any stair tread to an overhead obstruction was not at least 7 feet measured from the leading edge of the tread. (a) Fixture Mezzanine - Employees used a set of stairs to store and retrieve fixtures including "Alpha Towers". The overhead clearance at the top of the stairs was restricted to 3 feet 6 inches due to pipes running along the ceiling at this location, on or about June 6, 2012. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.303(g)(2)(ii): In locations where electric equipment was likely to be exposed to physical damage, enclosures or guards were not so arranged and of such strength as to prevent such damage: (a) Store Room, Mezzanine Light Fixtures - Employees worked to move merchandising equipment such as chrome "Alpha Towers" and other display equipment onto the mezzanine and were exposed to live parts of fluorescent light fixtures with missing bulbs and/or cover panels. The light fixtures with unprotected fluorescent bulbs were positioned in the ceiling as low as 6 feet 10 inches above the floor on the mezzanine level, on or about June 6, 2012. (b) Store Room, Light Fixtures Below Mezzanine - Employees worked to move merchandising equipment, boxes and other display equipment under the mezzanine and were exposed to live parts of fluorescent light fixtures with missing bulbs and/or cover panels. The light fixtures with unprotected fluorescent bulbs were positioned in the ceiling as low as 7 feet 3 inches above the floor below the mezzanine, on or about June 6, 2012. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
