29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level was not  guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder.    Employee(s), who access the tire elevated tire storage area were exposed to fall hazards of approximately 6 feet 6 inches from the working platform to the concrete below, in that a fall protection system was not in place to keep the employee from falling.

29 CFR 1910.177(c)(1): The employer did not provide a program to train all employees who service rim wheels in the hazards involved in servicing those rim wheels and the safety procedures to be followed.    On or about 06/14/2012, at a worksite located at 170th Ave and Hopewell Rd, Hays KS, an employee was exposed to crush-by and struck-by hazards in that a training program was not provide to minimize the hazards of servicing rim wheels to include the safe installation and removal of rim wheels, while working outside the facility in rural areas.

29 CFR 1910.244(a)(2)(iii): After the load has been raised, the load was not cribbed, blocked, or otherwise secured at once.    On or about 06/14/12, at a work site located at 170th and Hopewell Rd, Hays, KS, an employee was exposed to crush-by and struck-by hazards in that after jacking up the right rear of a 1970 C/50 Grain Truck with the use of two 20-ton air bottle jacks, the load was not immediately cribbed, blocked, or secured.

On or about 06/14/12, at a work site located at 170th and Hopewell Rd, Hays, KS, the bottle jacks in use did not have markings that were legible to indicate the load capacity.
