29 CFR 1926.1060(a):     The employer did not provide a training program for each employee using ladders and stairways, that enabled each employee to recognize hazards related to ladders and stairways, and that trained each employee in the procedures to be followed to minimize those hazards:    Worksite:  The employer did not provide ladder training to an employee under his control.    On or about 06/04/2012 an employee who was engaged in siding work fell off an aluminum extension ladder and sustain a broken leg.
