Location:  Location:  At the residence at or near 12128 Old St. Charles Road, Bridgeton, MO  63044.    On or about June 18, 2012, workers were using a pneumatic nailer without eye protection.

On or about June 18, 2012, workers were performing roofing activities on a roof with an approximate 6:12 pitch without any protection from falls when working at a height 6 ft. or greater.

29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    Location:  At the residence at or near 12128 Old St. Charles Road, Bridgeton, MO  63044.    The employer did not ensure that the ladder extended a minimum of 3 ft. above the roof.  The ladder was used by the workers to access the roof.
