All 120-volt, single-phase 15- and 20-ampere receptacle outlets on construction sites, which are not a part of the permanent wiring of the building or structure and which are in use by employees, shall have approved ground-fault circuit interrupters for personnel protection.     Location: 98 Clay Street, Brooklyn, NY    a)  Employees were using hand tools that were energized by portable electric generator that was not provided with a GFCI or an assured equipment grounding program. On or about 6/13/2012.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    Location: West of first floor.        a)  Employees were working near an open sided floor and were not protected with any form of fall protection. Employees were exposed to floor opening. On or about 6/13/2012.
