Warehouse and Shipping Docks - On or about June 19, 2012 employees were exposed to crushing hazards from two forklifts with ROPS installed and they were not protected by the installation of seatbelts on the Toyo Umpanke forklift (S/N 32000778) and Toyo Umpanke forklift (S/N 32000231) in the event of a tipover.
