29 CFR 1926.501(b)(13):  Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)           a.  On or about June 15, 2012, an employee working from the 4:12 pitched rooftop of a three story multi-family            building was not protected from a 27 feet fall hazard.    ABATEMENT DOCUMENTATION OR CERTIFICATION IS NOT REQUIRED FOR THIS ITEM.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      a.  On or about June 15, 2012, the ladder that was being used by an employee to again access to a roof landing was not positioned the minimum 3 feet above the eave or landing of the roof to provide the employee safe access and egress exposing him to a 27-foot fall hazard.    NO DOCUMENTATION OR CERTIFICAITON IS REQUIRED FOR THIS ITEM.
