29 CFR 1926.100(a):     Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    a) 28050 Hwy 28 in Hazlehurst, MS -- On or about June 20, 2012, an employee working on the ground next to the ladder and handing materials up or down to employees on the roof was not wearing a hard hat.

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) 28050 Hwy 28 in Hazlehurst, MS -- On or about June 20, 2012, the extension ladder used to access the roof was only extended 12 inches above the upper landing.
