29 CFR 1926.501(b)(10):  Employees on low-slope roof with unprotected sides and edges 6 feet (1.8 meters) or more above lower levels, were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:           On or about 6/1/12 emloyees were exposed to the hazard of falls from an elevation of approximately 12 ft. to the ground level below while installing a metal roof, without fall protection.

29 CFR 1926.503(a)(2)(iii):  The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used:               On or about 06/01/2012, employees that did not have fall protection training, were exposed to a fall hazard from an elevation of approximately 12 feet to the ground level below while installing a metal roof.

29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladders were not secured at the top to a rigid support, and/or a grasping device, such as a grabrail, was not provided to assist the employees in mounting and dismounting the ladder:              On or about 06/01/2012, employee(s) were exposed to the hazards of fall, when using an extension ladder that did not extended at least 3 feet above the landing.
