29 CFR 1910.134(c)(2)(ii):  Where respirator use was not required, the employer did not establish and implement those elements of a written respiratory protection program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use did not present a health hazard to the user:    a.  On or about June 21, 2012, an employee, finishing precast concrete work, was required to use a 3M 7500 Series Large half-face elastomeric facepiece respirator with acid/gas cartridges for acid wash finishing concrete operations, and the employer had not established nor implemented those elements of  a written respiratory protection program as required.      Abatement Certification and Documentation are required.
