29 CFR 1926.451(b)(1)(i):     Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) were not installed so that the space between adjacent units and the space between the platform and the uprights is no more than 1 inch (2.5 cm) wide.    On or about March 14, 2012, the employer did not ensure the planks were installed so that the space between them was no more than 1 inch wide exposing employees to a fall hazard.

29 CFR 1926.451(b)(7):     On scaffolds where platforms are overlapped to create a long platform, the overlap did not occur only over supports, and were not less than 12 inches (30 cm) unless the platforms are nailed together or otherwise restrained to prevent movement.    On or about March 14, 2012, the employer did not ensure the planks of the scaffold platform were placed only over supports and the planks were not less than 12 inches over the area exposing employees to a fall hazard.

29 CFR 1926.451(g)(4)(i):     Guardrail systems were not installed along all open sides and ends of platforms. Guardrail systems were not installed before the scaffold is released for use by employees other than erection/dismantling crews.    On or about March 14, 2012, the employer did not ensure a guardrail system was installed along all open sides of the scaffold platform exposing the employees to a fall hazard of more than 10 feet to the ground below.

29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.     On or about March 14, 2012, the employer did not ensure employees working from the third level balcony of a building were in use of fall protection exposing the employees to a fall hazard of more than 6 feet to the ground below.
