29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:    On or about June 19, 2012, an employee working on a boom ladder at a commercial building located at 9328 Dyer El Paso, TX, was not wearing any type of fall protection. This condition exposed the employee to falls of approximately 20 feet to the ground level.
