Instance c) In the Buffer and Shipping Department the Pedestal, Electronic, Variable Speed Buffer and Polisher did not have guards installed over the 18 inch buffing pads.   Instance d). In the Buffer and Shipping Department the Jet 10" Buffer/Sander Model# JSB-10H did not have guards installed over the 10 Inch buffing pads.   Instance e) In the Press Break area the blue Alantic Machine Tools Press Break, Model HDC was not equipped with machine guards to prevent employees' hands from being caught in the machine.   Instance f) In the Press Break area the grey Haco Machine Tools Press Break was not equipped with machine guards to prevent employees' hands from being caught in the machine.

Bandsaw wheels were not fully encased. The front and back of the band wheels were not either enclosed by solid material or by wire mesh or perforated metal. Such mesh or perforated metal was not not less than 0.037 inch (U.S. Gage No. 20), and the openings were greater than three-eighths inch. The guard for the portion of the blade between the sliding guide and the upper-saw-wheel guard did not protect the saw blade at the front and outer side.  On or about June 08, 2012, and at times prior thereto, the Delta Band Saw located in the shipping department did not have guards installed over the non-working portion of the blade and the band saw's wheels were not fully encased with solid material or by wire mesh or perforated metal.
