29 CFR 1926.451(g)(1)(vii):     Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level.  Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold.     For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    a.  At the site, employees were installing vinyl siding on the back of a residential home from a pump jack scaffold that did not have a midrail, screen, or mesh installed to prevent the employees from a fall, thereby exposing the employee to a fall hazard of 13-1/2 feet.

When the pump jack has to pass bracing already installed, an additional brace shall be installed approximately 4 feet (1.2 m) above the brace to be passed, and shall be left in place until the pump jack has been moved and the original brace reinstalled.    a. 29 CFR 1926.1053(b)(1):     When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.    a.  At the site, employees were installing vinyl siding on the back of a residential home from a pump jack scaffold.  The primary means of accessing the scaffold was by a portable ladder that did not extend at least three feet above the scaffold platform, thereby exposing the employee to a max fall hazard of 13-1/2 feet.
