29 CFR 1926.307(c)(4)(i): Projecting shaft ends did not present a smooth edge, projected more than one-half the diameter of the shaft, and were not guarded by non-rotating caps or safety sleeves:    a) For the construction site at 3501 E. Hillsborough Ave., Tampa, FL 33610, as observed on or about 06/18/2012: employees were exposed to an unguarded rotating shaft on the motor for a 5500W generator. The rotating shaft, used as a pull starter, had sharp edges and grooves on the metal that could lacerate employees hands. Clothes could also get caught on the shaft. The length of the shaft was approximately equal to the diameter.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) For the construction site at 3501 E. Hillsborough Ave., Tampa, FL 33610, as observed on or about 06/18/2012: employees were exposed to fall hazards of up to 25 ft. while performing roofing work on a 52.3 degrees pitched roof (pitch= 15:12). No fall protection was available or being used. Employees were able to balance and not slide off by bracing their feet against the top of an extension ladder that extended partway up the roof and contoured the pitch of the roof.

Hillsborough Ave., Tampa, FL 33610, as observed on or about 06/18/2012: employees were working at the edge of a roof off of metal, conductive ladders near the power line that supplied power to the restaurant. Non-conductive side rails for the ladders were not present to protect the employee working on the ladder and the employee holding or adjusting the ladder at ground level.
