29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    On or about June 18, 2012, in the automotive shop:    Employees performing automotive maintenance and repair work were exposed to struck-by and amputation hazards when the employer did not develop a hazardous energy control program (lockout/tagout) that contains, written specific procedures, periodic inspection, and training, for the employees working on automobiles.

29 CFR 1910.145(f)(3): Tags were not used as a means to prevent accidental injury or illness to employees who were exposed to hazardous or potentially hazardous conditions, equipment or operations which were out of the ordinary, unexpected or not readily apparent.     On or about June 18, 2012, in the automotive shop:    Employees performing automotive maintenance and repair work were exposed to amputation hazards when the employer did not use tags to warn employees when equipment that includes but is not limited to automotive lifts and tire changing machines is unsafe and should not be used.
