29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a) Construction Site: On or about June 29, 2012, and at times prior thereto, employees were exposed to fall hazards of approximately 17 feet while accessing a residential roof with a portable extension ladder that exceeded the eave by approximately 1 foot and was not tied off.

(a) Construction Site: On or about June 29, 2012, and at times prior thereto, employees were exposed to fall hazards while using a portable extension ladder and were not provided with adequate training on the proper use of portable extension ladders.
