29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a)     On or about June 26, 2012, the exposing employer, Masterteam LLC, did not ensure employees, who were exposed to overhead hazards while engaged in re-roofing operations, were protected from head injuries due to possible falling objects.

Note:  If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502:    a)     On or about June 26, 2012, the exposing employer, Masterteam LLC, did not ensure employees were adequately protected from fall hazards in excess of 10 feet while engaged in re-roofing operations.
