29 CFR 1910.147(c)(5)(ii)(B):  Lockout and tagout devices were not standardized within the facility in at least one of the following criteria: color; shape; or size:    (a)  Various sites - Employees required to perform work on roofs, where electrical shut offs were located on a different level were not provided with specific energy control devices, on or about June 28, 2012.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices:    (a) Various Sites - Employees required to perform work on roofs, where electrical shut offs were located on a different level were not provided with adequate training.  The employer did not ensure that employees had the knowledge and skills to safely apply, use and remove energy control devices, on or about June 28, 2012.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
