29 CFR 1926.453(b)(2)(iv): Employees shall always stand firmly on the floor of the basket, and shall not sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position.    WORKSITE: The employee climbed from an open side of the porch roof over the railing of the Sky Power, model 600SJ aerial lift was not wearing a safety harness exposing him to approximately a 12 foot fall.

29 CFR 1926.453(b)(2)(v): A safety harness shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).    WORKSITE: The employee installing a metal fa�ade on the side of the building, 17 feet above the ground from the Sky Power, model 600SJ aerial lift was not wearing a safety harness.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    WORKSITE: The employee installing metal faade from the porch roof were not protected from falling approximately 12 feet  by the use of appropriate fall protective devices and/or systems.     Blair Companies Inc., was previously cited for a violation of an equivalent occupational safety and health standard 29 CFR 1926.501(b) (10), which was contained in OSHA inspection number 312552599, citation number 1, item number1, issued on 7/31/2009, and affirmed as a final order on 8/10/2009 with respect to a workplace located at 4928 Main Street, Steven Point WI, 54481.
