29 CFR 1926.451(b)(1)(i):     Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.      a) On or about 07/03/12, on construction site at 3755 Harlem Rd., Cheektowaga, NY:  Two mobile fabricated frame scaffolds were erected on the south side of the building. One employee installing replacement lentels while working from the top level of each scaffold was exposed to a fall of approx. 15 ft. because the level was not fully planked. The width of each scaffold was 60 inches, and there were only two approx. 19 in. wide "piks" in place on each scaffold instead of the three required.      NO ABATEMENT CERTIFICATION REQUIRED
