29 CFR 1926.453(b)(2)(v):     A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    A) On or about 06/28/2012 during the installation of the metal decking at New Life Tabernacle Church, Seffner Fl, an employee was observed being raised in an aerial lift basket. The employee that had no fall protection was exposed to heights of approximately 11 feet.

29 CFR 1926.760(a)(1):     Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.    A) On or about 06/28/2012, the employees at the New Life Tabernacle Church, Seffner Fl engaged in the installation of the metal decking without the establishment of a controlled decking zone (CDZ) were observed without fall protection and exposed to heights of approximately 17 feet.
