29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.    On or about July 6, 2012 employees were working below other workers, tools and materials without hard hats, exposing employees to overhead struckby hazards.      Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems:    On or about July 6, 2012 employees were working on a roof in a hoist area lowering materials off of the roof without fall protection, exposing the employees to fall hazards of 21.2 feet.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
