On or about 06/29/12, at the site of 64-33 232nd Street. Oak Garden, NY Employees working on the ground were in danger of head injury from falling objects such as, but not limited to tools and roof materials from the roof. Protective helmets were not worn. ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19

Oakland Gardens, NY Employees replacing new roof on a pitch roof with a slope of 6 in 12 ratios were exposed to the hazard of falling approximately 20 ft to the ground below. ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.

Oakland Gardens, NY Employees were using a portable ladder for gain access to a roof. The ladder side rails extended one foot above the upper landing surface. The employees were exposed to the hazard of falling approximately 20 ft to concrete ground below. ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.
