29 CFR 1910.212(a)(5): Fan blade guard(s) were not provided where the periphery of the blades was less than seven feet from the floor or working level: (a) In the laundry room behind the driers: On or about July 3, 2012; the employer allowed employees accessing the area behind the driers to be exposed to struck-by hazard due to the periphery of the fan being unguarded at a height of approximately 48 inches above the floor.

29 CFR 1910.303(g)(1)(i)(B): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the working space did not permit at least a 90-degree opening of equipment doors or hinged panels: (a) In the laundry room on the wash side: On or about July 3, 2012; the employer did not ensure that areas in front of electrical disconnect panels were maintained free of obstruction with a space at least 30 inches to allow access to the panel for servicing. (b) In the maintenance room behind the ice machine: On or about July 3, 2012; the employer did not ensure that areas in front of electrical disconnect panels were maintained free of obstruction with a space at least 30 inches to allow access to the panel for servicing.
