29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    a) Penn Treaty Village, Roof - Employees engaged in roofing activities were exposed to fall hazards ranging from ten (10) to ninety (90) feet without the benefit of conventional or alternative methods of fall protection.  Observed on or about June 14, 2012.

29 CFR 1926.503(a)(1):     The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.    a) Mazzoni Brothers Roofing - The employer did not provide training to all employees potentially exposed to fall hazards.  Two employees performing roofing activities were exposed to fall hazards ranging from ten (10) to ninety (90) feet.  Observed on or about June 14, 2012.

29 CFR 1926.20(b)(1):     It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.    a) Mazzoni Brothers Roofing - The employer did not develop and implement a written safety and health program to assist employees with the hazards associated with their operations.  Observed on or about June 14, 2012.
