29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) North exterior side of the structure at the secondary entrance; On or about July 10, 2012 and at times prior thereto, an employee installing aluminum coping on a parapet was working from the roof deck without a means of fall protection. Note: Abatement certification is NOT required for this item.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) North exterior side of the structure at the secondary entrance; On or about July 10, 2012 and at times prior thereto, an employee installing aluminum coping on a parapet did not extend the ladder 3-feet above the roof deck before accessing it. Note: Abatement certification is NOT required for this item.
