29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:    (a)  3505 Gettysburg Road, Camp Hill, Pennsylvania - Employees used scaffolding with platforms for accessing the upper levels.  The platform of the walkways was not at least 18 inches wide, on or about June 29, 2012.      Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.    (a)  3505 Gettysburg Road, Camp Hill Pennsylvania - Employees worked on the side bracket on the third buck of a fabricated frame scaffolding.  The employer did not insure that the scaffolding was provided a mid-rail, on or about June 29, 2012.      (b)  3505 Gettysburg Road, Camp Hill Pennsylvania - Employees used a platform to access the side bracket.  The employer did not insure that the scaffolding was provided with a mid-rail, on or about June 29, 2012.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
