29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two:    (a) BFC Industrial Services, LLC DBA Atlas Rigging & Transfer at 3900 Joliet St. in Denver, CO: On July 12, 2012 horizontal lifelines were not designed, installed, and used under the supervision of a qualified person.  Two employees working on a pipe rack on the East end of the facility were tied off to a nylon rope that was not engineered to be a horizontal lifeline.  This condition exposed the employees to a fall hazard of 17' from the pipe rack to the floor of the tank farm.    Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

This condition exposed employees to a respiratory hazard.    Abatement Note:  This evaluation shall include a reasonable estimate of employee exposures to respiratory hazard(s) and an identification of the contaminant's chemical state and physical form. Where the employer cannot identify or reasonably estimate the employee exposure, the employer shall consider the atmosphere to be IDLH.    Abatement Note: The requirements of 1926.103  for respiratory protection for construction work are identical to those set forth in 1910.134.    Abatement Note:  Abatement certification is not required for this item.
