29 CFR 1910.147(c)(7)(i)(A):     Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.    a) On or about 7/10/12 in the shipping area; employer did not provided the required training to employee(s) involved in servicing activities, such as but not limited to, the die changes on the Bliss power presses, so that locks were applied when performing this  activity. 29 CFR 1910.147(d)(4)(i):     Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees.    a) On or about 7/10/12 in the Press area; during die changing on the Bliss power presses ( BPM-11 & 12 ) locks were not attached to the energy isolation devices as described in the PGM Bliss BPM-12 LOTO written procedure.
