29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.    a) Fire Station - Employees were working on a scaffold system three bucks high and approximately 18 feet without fall protection such as but not limited to a guardrail system installed along all open sides and ends of the platforms.  Employees were exposed to a fall hazard.

29 CFR 1926.452(c)(2): Frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which would secure vertical members together laterally, and the cross braces were not of such length as would  automatically square    a) Fire Station - Employees were working from a scaffold system where cross bracing had not been installed on all sections of the scaffold system to prevent displacement.  Two sections had one side cross braced.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.    a) Fire Station - The employees working on the scaffold had not been trained by a competent/qualified person on recognizing fall hazards while working on scaffolds.  The employee was exposed to a fall hazard approximately 18 feet above ground level.
