Richmond Machine Company, located at 1528 Travis Dr. Montpelier, OH 43543; On or about July 11, 2012, the employer did not assure energy control procedures were utilized and developed for safe performance of service and maintenance. Employees were exposed to caught by and/or amputation hazards from the point of operation while performing set up activities on the Fadal CNC 88HS, Fadal VMC 6030D, and Fadal CNC VMC 4020 CNC Machines.    b.) Richmond Machine Company, located at 1528 Travis Dr. Montpelier, OH 43543; On or about July 11, 2012, the employer did not assure energy control procedures were utilized and developed for safe performance of service and maintenance. Employees were exposed to caught by and/or amputation hazards from the point of operation while cleaning metal chips and oil residue in between cycles on the Fadal CNC 88HS, Fadal VMC 6030D, and Fadal CNC VMC 4020 CNC Machines.

29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a.) Richmond Machine Company, located at 1528 Travis Dr. Montpelier, OH 43543; On or about July 11, 2012, the employer did not assure employees were protected from amputation hazards while running the Fadal CNC 88HS and Fadal VMC 6030D CNC Machines. Employees use the CNC machines to bore and mill various metal parts, and are exposed to potential amputation hazards during the cycle from inoperable interlocks or inadequate guarding.    b.) Richmond Machine Company, located at 1528 Travis Dr. Montpelier, OH 43543; On or about July 11, 2012, the employer did not assure employees were protected from caught by hazards from the  Bridgeport Drill Press. Employees use the Bridgeport Drill Presses for various milling activities and are exposed to caught by hazards from the unguarded chuck.    c.) Richmond Machine Company, located at 1528 Travis Dr. Montpelier, OH 43543; On or about July 11, 2012, the employer did not assure employees were protected from caught by hazards while using the C10TM Lion Engine and Lansing 36 Lathes. Employees were exposed to the unguarded rotating chucks while milling various metal parts.
