29 CFR 1926.1053(b)(13):     The top or top step of a stepladder shall not be used as a step:    a.  On or about July 10, 2012, an employee was using the top step of a 4-foot fiberglass Werner stepladder while installing drywall, exposing himself to a 4 feet fall hazard.

29 CFR 1926.1060(a):     The employer did not provide a training program for each employee using ladders and stairways, as necessary. The program did not enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards:    a.  On or about July 10, 2012, employees were using a stepladder while installing drywall and had not received training about the hazards associated with using ladders.
