29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      On or about July 13, 2012, employees were climbing a ladder that did not extend at least 3 feet above the landing to gain access to the upper landing to place shingle bundles on the roof. Employees were exposed to an unprotected fall of over 20 feet to the ground below.

29 CFR 1926.1053(b)(22): Employees  using ladders were carrying objects or loads that could cause the employee to lose balance and fall:      On or about July 13, 2012, employees were using ladders while carrying bundles of shingles on their shoulders and exposed to an unprotected fall of over 20 feet to the ground below.
