29 CFR 1910.303(g)(1)(i)(A):     For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910:    (a) The employer does not ensure that electrical equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts was not less than indicated in Table S-1 of Subart S of Part 1910.  This violation occurred on or about July 13, 2012 when the main power inlet panels in the warehouse had only sixteen (16) inches of clearance where three (3) feet were required as indicated in Table S-1 of Subpart S of Part 1910.

29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.    a)  The employer does not ensure that flexible cords and cables are connected to fittings so that strain relief is provided that will prevent pull from being directly transmitted to the joint or terminal screws.  This violation occurred on or about July 13, 2012 in the liquid filling area when the power cable for the scale (Michellli Scales Model QC3265) was damaged from strain at the plug end of the cord.

29 CFR 1910.304(g)(5):     The path to ground from circuits, equipment, and enclosures was not permanent, continuous and effective:    (a)  The employer does not ensure that the path to ground from circuites, equipment, and enclosures is permanent, contiunous and effective.  This violation occurred on or about July 13, 2012 when a metal Michelli Scale (Model QC-3265) in the liquid filling area did not have a continuous path to ground.
