29 CFR 1910.146(c)(2):     The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces:     On or about July 16, 2012, at the receiving pit of Silo #1,  employees were exposed to engulfment and caught-in hazards when the employer failed to effectively communicate the danger posed by entering the receiving pit.

29 CFR 1910.272(e)(1)(ii):     Current employees, and new employees prior to starting work in the grain handling facility were not trained in the specific procedures and safety practices applicable to their job tasks including but not limited to cleaning procedures, cleraing procedures, housekeeping procedures, hot work procedures, preventive maintenance procedures and lock-out/tag-out procedures:    On or about July 16, 2012, receiving pit of Silo 1, all employees performing grain handling work were not effectively trained in safe bin entry procedures, lockout procedures, and rescue procedures, exposing employees to engulfment and caught-in hazards.
