This violation occurred on July 19, 2012, at a work place located at 5508 Village Green, Mesquite, TX; employees were not using fall protection while performing roofing work and were exposed to falling to the lower level.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access:    This violation occurred on July 19, 2012, at a workplace located at 5508 Village Green, Mesquite, TX; employees were using a 20-foot aluminum extension ladder that did not extend 3-feet above the landing of the roof.
