29 CFR 1910.176(b): Secure Storage. Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse:    (a) HonAccurate @ 1330 Main Street, Longmont, CO:  Mechanic Shop. On or before July 6, 2012, the employer did not ensure the rows of car tires stored in an upright position on the top tier of the metal rack were securely fastened or limited in height to prevent them from falling. The metal rack system was not fastened or secured against sliding and collapse. This condition exposed employees to the hazard of being struck by unstable car tires and other materials.

29 CFR 1910.305(d)(2): Panelboard enclosures. Panelboards shall be mounted in cabinets, cutout boxes, or enclosures designed for the purpose and shall be dead front.     (a) HonAccurate @ 1330 Main Street, Longmont, CO:  Mechanic Shop. On or before July 6, 2012, the employer did not ensure the electrical panel locate at the north wall of the building was protected with an approved cover or enclousre so that electrical contact with  exposed live electrical parts could be prevented.  This condition exposed employees to an electrical hazard.

OSH ACT of 1970 Section (5)(a)(1): of the Occupational Safety and Health Act of 1970: the employer did not furnish to each of his employees employment and a place of employment which are free from recognized hazards that are causing or likely to cause death or serious physical harm to his employees, in that employees were exposed to being struck-by and/or crushed by an automobile lift, which was not installed according to the manufactures instructions. The Installation Report was not completed by the employer nor was a copy sent to the lift manufacturer for its approval.     HonAccurate @ 1330 Main Street, Longmont, CO:  Mechanic Shop. On or before July 6, 2012, the employer did not ensure that employees were protected from strike-by and/or crushing hazards while using the automobile lift. The Installation Report from Auto Lifters of America, Model 9000 was not completed by the employer nor was a copy sent to the lifts manufacturer for its approval as the final authority in all matters pertaining to the installation of the lift.  (1)  The hazard of being struck-by and crushed while elevating vehicles using the Auto Lifters of America, Model, 9000, which was not installed according to the manufactures instructions. The Installation Report was not completed by the employer nor was a copy sent to the lift manufacturer for its approval as the final authority in all matters pertaining to the installation of the lift.    Abatement Note:  Abatement certification is required for this item (see enclosed Abatement-Certification Letter").

29 CFR 1910.1200(e)(1):  The employer did not develop, implement, and maintain a written hazard communication program which at least described how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met, and which also included a hazardous chemicals list and methods used to inform employees of the non-routine tasks:    (a) HonAccurate @ 1330 Main Street, Longmont, CO:  Mechanic Shop. On or before July 6, 2012, the employer did not ensure that a written hazard communication program which at least described how the criteria specified in for labels and other forms of warning, material safety data sheets, and employee information and training will be met, and which also included a hazard chemicals list and methods used to inform employees of the hazards of routine tasks, had been developed for employee exposures, such as but not limited to the following:             (1)  Brake Fluid;  (2)  Brake Cleaner;  (3)  Carburetor Cleaner;  (4)  Motor Oil.

On or before July 6, 2012,  the employer did not ensure that employees were provided with effective information and training on hazardous chemical their work area at the time of their initial assignment, and whenever physical or health hazard is introduced into their work area, such as but not limited to the following chemicals:    (1)  Brake Fluid;  (2)  Brake Cleaner;  (3)  Carburetor Cleaner;  (4) Motor Oil.
