OSH ACT of 1970 Section (5)(a)(1): of the Occupational Safety and Health Act of 1970: the employer did not furnish to each of his employees employment and a place of employment which are free from recognized hazards that are causing or likely to cause death or serious physical harm to his employees, in that employees were exposed to being struck-by and/or crushed by an automobile lift, which was not installed according to the manufactures instructions. The Installation Report was not completed by the employer nor was a copy sent to the lift manufacturer for its approval.     HonAccurate @ 1330 Main Street, Longmont, CO:  Mechanic Shop. On or before July 6, 2012, the employer did not ensure that employees were protected from strike-by and/or crushing hazards while using the automobile lift. The Installation Report from Auto Lifters of America, Model 9000 was not completed by the employer nor was a copy sent to the lifts manufacturer for its approval as the final authority in all matters pertaining to the installation of the lift.  (1)  The hazard of being struck-by and crushed while elevating vehicles using the Auto Lifters of America, Model, 9000, which was not installed according to the manufactures instructions. The Installation Report was not completed by the employer nor was a copy sent to the lift manufacturer for its approval as the final authority in all matters pertaining to the installation of the lift.
